Chartnote AI Scribe records patient encounters and turns conversations into structured clinical notes for review before transfer to an EHR. It creates SOAP notes with assessment and plan sections, and supports custom templates and style controls. The process is to record, generate, then review and finalize the note. Documentation tools include real-time dictation, Copilot assistance, and reusable snippets. All plans include more than 1,000 preloaded templates and access to a community template library. Its Chrome extension can dictate, display notes and snippets, and send documentation into web-based EHRs. Intelligent Mapping links note sections such as HPI, ROS, Assessment, and Plan to corresponding EHR fields for one-click transfer. The mobile app can serve as a microphone for Chartnote or the extension. Chartnote states that it undergoes an annual independent SOC 2 Type II examination and maintains HIPAA, GDPR, PIPEDA, and CCPA compliance programs. It also says patient information and clinical content are not used to train AI models by Chartnote or its providers. Users can delete notes and recordings manually or configure automatic deletion after 1–30 days.
